The "In-Game Purchases" label is the item most likely to be misread. It does not confirm microtransactions in the single-player game.
Rockstar said earlier this summer that GTA VI has "no monetizations in this game." The ESRB notice is a standard disclosure that a title offers something purchasable inside it, and the most likely candidate is the paid Ultimate Edition upgrade, which unlocks extra cosmetics, stores and missions.
